Sports Ministry Policy

PARENT/GUARDIAN AUTHORIZATION: I certify that I have received any, and all information that I deem necessary for my child to participate in the Rock Springs Sports Ministry activity identified on this form. Also, I agree to assume all risks of his/her participation in the activity and agree to hold harmless Rock Springs Church, its employees, or volunteers against any liability claims, damages, or expenses arising from my child’s participation in the Rock Springs Sports Ministry activity. I consent to photos and videos being taken to be used for marketing and promotion purposes of Rock Springs Sports Ministry and Rock Springs Church. Finally, I authorize the staff of the Rock Springs Sports Ministry activity to secure any emergency medical treatment necessary to treat my child in the event that it is deemed necessary by the staff of the Rock Springs Sports Ministry activity.
